Payer contract review for New York City medical practices

For a medical practice in New York City, a useful commercial payer review starts with the executed fee schedule, participating network and actual service location. NPAG can help identify contract priorities and the evidence needed for a rate request; a benchmark gap is not a guaranteed increase.

Review borough-level service locations and network participation

For New York City practices, including Brooklyn offices, a single city label can hide several service locations. Identify each site, county and participating network when checking the CMS locality key or a commercial exhibit. Do not import an upstate reference into a downstate comparison without verification.

  • Are Manhattan, Brooklyn or other borough sites covered by the same agreement and rate exhibit?
  • Does each provider participate in the same plan product at each site?
  • Does the source comparison match the applicable locality and service year?

Prepare your New York City reimbursement baseline

Group documents by payer product and participating site, not just payer name. Use DFS provider guidance to distinguish coverage and complaint questions from a request for new commercial rates.

Use permitted, deidentified summaries rather than patient-level records. Gather signed agreements, amendments, annual code volumes and applicable renewal deadlines. You do not need to upload these documents to request an initial conversation.

Provider resources in New York

New York City and Rochester should not be treated as one reimbursement market simply because both are in New York State. Identify the service location, payer product and network, then check the relevant schedule. DFS provides provider guidance; whether a state process applies depends on the coverage and issue.

Confirm the applicable plan, regulator and dispute process before relying on state guidance. Payment enforcement and a request for better future contract rates are different tasks; neither establishes an entitlement to a rate increase.

What a local comparison does not establish

Our online references are national screening figures—not verified local, state-specific or payer-specific offers. Match the code, modifier, date, network and professional or facility setting before describing a benchmark as local. Unsupported codes remain unbenchmarked.

Commercial contracts more than five years old are eligible for renegotiation screening, subject to their actual terms. A payer may decline a request. An opportunity identified, a signed rate amendment and payments verified after implementation are separate outcomes.
